Hospice is a form of care tailored specifically for the needs of individuals and families facing a life-limiting illness. A diverse medical care team works together to manage pain, increase comfort, and support every aspect of a patient’s physical, emotional, and spiritual health. The goal of hospice care is to help people live their last days with respect, peace, and dignity.

Our Philosophy

Hospice affirms life. Hospice exists to provide support and care for persons in the last phases of a life-limiting illness so that they may live as fully and comfortably as possible. Hospice exists in the belief that through appropriate care and the promotion of a caring community, patients and families/caregivers will be free to attain a degree of mental preparation for death that is satisfying to them.

Indicators that a patient is hospice-eligible can include but are not limited to:

  • Weight Loss
  • Increased assistance with daily activities
  • Increased weakness and fatigue
  • Frequent and multiple falls
  • Difficulty swallowing
  • Shortness of breath or chest pain
  • Continuous oxygen
  • Increase/changes in medication
  • Recurring or multiple infections
  • Frequent phone calls to physician
  • Frequent hospitalizations or ER visits
